Learning objectives:

• Facilitate multidisciplinary discussion of cases of potential or real adverse patient events  to identify areas for improvement
• Review systems based issues, knowledge gaps, or biases that can lead to potential or real adverse  patient events.
• Stimulate members of the department of Pediatric and Adolescent Medicine to implement action plans to prevent similar events from occurring with future patients.
