DESCRIPTION
The recurring Morbidity and Mortality (M & M) Meetings provide a structured, case-based educational forum for emergency medicine clinicians to review adverse events, near misses, and unexpected outcomes occurring in the emergency department. Each session involves a detailed analysis of clinical care, contributing factors, and system-level processes that influenced the outcome.
Live-Streamed; Monthly on the Second Wednesday from 8:30-9:30 a.m.

Nurse Practitioners, Pharmacists, Physicians, and Physician Assistants
LEARNING OBJECTIVES
Upon conclusion of this activity, presenters should be able to:
- Review selected emergency medicine cases brought forward by clinical providers, the legal/risk management team and patient experience representatives identifying key clinical decisions and system factors.
- Diagnose contributing factors (e.g., cognitive errors, communication gaps, system failures or procedural deviations) that influenced patient outcomes in each case.
- Discuss evidence-based principles and current clinical guidelines to evaluate the appropriateness of care delivered and identify opportunities for alternative approaches.
- Apply multidisciplinary discussion to encourage shared learning, promote a culture of safety, and gain diverse perspectives on clinical decision-making and system processes.
- Identify and prioritize specific, actionable improvement opportunities aimed at reducing preventable harm and enhancing patient safety in the emergency department setting.
- Arrange follow up actions, including quality improvement initiatives, protocol updates, or targeted education, as appropriate based on the case analysis.
