The last major changes in key concepts of mental illness as well as pharmacologic and psychotherapeutic treatments occurred in the 1980s and 1990s with the advent of the modern era of DSM diagnostic classification, approvals of second generation psychotherapeutics, and widespread adoption of CBT and third wave psychotherapies. This presentation will raise the possibility that the 2020s and 2030s portend another era of transformative change in psychiatry and psychology. (1) Emerging knowledge about brain functioning from networks to neurons will complement current data on brain structure. Concepts of mental illness arising from alterations in the functioning of brain networks will complement those grounded in neurochemistry. (2) Portable, wearable, and implantable devices will become essential for advanced diagnostics and therapeutics. (3) Virtual technologies will individualize psychotherapy and extend the reach of therapists to people who currently have no real access to care. (4) The electronic medical record will be a repository of data streams accessible to patients and clinicians to predict and manage illnesses more precisely.
