Upon conclusion of this activity, participants will be able to:

  • Summarize what the current process is for result follow-up for patients discharged from the hospital.
  • Validate the challenges associated with transitioning care from inpatient to outpatient settings. 
  • Validate the collaboration that transitions of care requires.
