This PMR Grand Rounds series on falls will utilize a wide lens in exploring the diverse realms of falls. This course series aims to provide a look into the multi-disciplinary services Mayo Clinic is currently providing in the realm of Falls and spur discussion on what can be done to further evidence-based prevention and treatment of falls across the enterprise. Experts from multiple disciplines including MD, RN, PT, and OT, will encompass the various stakeholders involved in identifying and treating patients at risk for falls. The topics covered in this series will include etiology of falls, identification of patients at risk for falls, proper referral for treatment services, measures to prevent falls in the inpatient care environment, and screening tools/assessments and treatment for patients at risk for falls with various diagnoses and comorbidities within a wide range of settings including skilled nursing/assisted living, inpatient, outpatient, and the community.

1. Outline the etiology of falls.
2. Identify fall risk factors in patients with various diagnoses and comorbidities.
3. Discuss screening tools to identify fall risk for individuals in the geriatric population.
4. Describe treatment strategies to reduce fall risk for individuals in the geriatric population.
5. Recognize importance of an interdisciplinary, collaborative approach to prevent falls in the acute care setting.
6. Discuss patient identification and safe patient handling measures to reduce the rate of falls in hospitalized patients.
7. Discuss screening tools to identify fall risk of individuals with neurological diagnoses.
8. Discuss treatment strategies to reduce fall risk for individuals with neurological diagnoses.

Mayo Faculty
Kim Seidel-Miller MD - Dr. Seidel is a consultant in PM&R. Her clinical practice areas include both inpatient and outpatient spinal cord injury rehabilitation as well as general neurologic rehabilitation in the outpatient setting. In all settings, she works alongside therapy to identify risks for and underlying cases of falls as well as to provide adaptations to reduce falls.
Will Wallstrom PT, DPT, GCS - Dr. Wallstrom is a Physical Therapist at the Mayo Clinic in Arizona. He received a Doctor of Physical Therapy degree from Franklin Pierce University in Goodyear, Arizona in 2013, and in June of 2019 became board certified as a Geriatric Clinical Specialist. He serves as a mentor for the geriatric and acute residency program and a member of the Arizona Falls Prevention Coalition. His clinical expertise is in managing the geriatric population across practice settings including home health, outpatient, and subacute inpatient rehabilitation.
Erin Richardson OT, OTD - Dr. Richardson is an occupational therapist specializing in geriatric care, currently working PRN at Mayo Clinic and at Sun Health senior services, with an emphasis on working with the dementia population. She is certified in dementia-capable care and has clinical expertise working with elderly populations in settings such as home health, post-acute care, community-based services, and driver's rehabilitation, and is a member of the Arizona Falls Prevention Coalition
Caitlin Polley PT, DPT, CCS - Caitlin is the Acute Care PT Hospitalist Lead in Rochester. She has been involved in multiple enterprise and institutional committees related to falls and safe patient handling. Caitlin is the safe mobility champion representing PMR therapy for Rochester. She participates in root cause analyses for falls with injury as a therapy representative. Caitlin is passionate about activity and mobility promotion for decreased functional decline and prevention of falls in the inpatient setting.
Katie Bauer MS, OTR/L, BCPR - Katie is a Hospitalist Lead in the Department of Physical Medicine and Rehabilitation at Mayo Clinic - Rochester. She received her Master of Science in Occupational Therapy from UW-LaCrosse and has her AOTA Board Certification in Physical Rehabilitation. She has over 12 years of experience in acute care. Katie is a Gold-certified Mayo Quality Fellow, Epic superuser, and represents OT on the Safe Patient Handling and Mobility Subcommittee, PMR Documentation Committee, Mental Health Quality Group, and List Coordination.
Heather Ducharme PT, DPT, ATC, GCS - Heather is a physical therapist on Mayo 14, primarily seeing patients with neurodegenerative conditions and functional movement disorders. She also has additional interests in treating older adults, and recently became a geriatric clinical specialist. Her practice is grounded in finding what brings patients joy and using their motivators to get them moving and grooving.
Kate Weber OTD, OTR/L - Kate grew up in Charlotte, North Carolina where she completed her undergraduate degree at UNC Charlotte. She then obtained her doctorate in Occupational Therapy at MGH Institute of Health Professions in Boston, Massachusetts. Following her love for neurorehabiliation, she completed the Occupational Therapy Neurorehabiliation Fellowship program at Mayo Clinic in Rochester. She has a passion for neurorehabiliation and works in the acute, inpatient, and outpatient clinical settings.
