Travel
The Ritz-Carlton, Kapalua is situated on the northwestern tip of the island of Maui. The resort is 45-60 minutes from the Kahului Airport (OGG) and five minutes from the Kapalua Airport, which services inter-island flights only. 

The Ritz-Carlton, Kapalua does not provide complimentary shuttle service from Kahului Airport (OGG), but shuttle service arrangements may be made for a fee by contacting the hotel at (808) 665-7089. There is a complimentary shuttle to and from Kapalua Airport (JHM) to The Ritz-Carlton, Kapalua.

Taxis are easily accessible at the Kahului Airport (OGG) airport. A one-way taxi fare to The Ritz-Carlton, Kapalua is approximately $130 to $150 USD.

Eight major rental car companies are represented at the Kahului Airport (OGG). Enterprise Rent-A-Car is available on-site at The Ritz-Carlton, Kapalua and can be arranged through Enterprise Rent-A-Car or the resort concierge for pick-up and drop-off at either Kahului Airport (OGG), Lahaina Town or at the hotel. Advance reservations are suggested by calling Enterprise at (808) 669-1901.

The above rate is per room per night, plus tax. There is also a reduced daily resort fee of $15 USD (plus tax) per room which covers: wireless high-speed Internet access, prevailing hotel golf fees and preferred tee times at The Bay and The Plantation Golf Courses within the Kapalua Resort, Resort shuttle services (Kapalua Resort and West Maui Kapalua Airport only), use of the 24-hour fitness center, and local and toll-free telephone calls.  

Reservations can be made online or by calling 1-800-262-8440. Please identify yourself as a participant in the Mayo Clinic Neuroradiology course when making your reservation. The special group rate is offered November 5, 2015, through November 15, 2015, based on availability. In order to receive the special group rate, reservations must be made prior to Wednesday, October 7, 2015, and will be based on availability. Early reservations are highly encouraged to avoid disappointment. Reservations will be taken following this date based on space and rate availability.

Hotel check-in time is 4:00 p.m. on the day of arrival, and check-out time is before 12:00 p.m. on the day of departure.

Mayo Clinic Department of Radiology CME Office is not responsible for expenses incurred by an individual who is not confirmed as a meeting participant and for whom space is not available at the meeting. Costs incurred by the registrant such as airline or hotel fees or penalties are the responsibility of the registrant.

Valet parking is $18 USD per day and self-parking is $10 USD.

Syllabus
A complimentary syllabus on flash drive is provided for this course. A paper syllabus is not available.  Also, a mobile app for both Android and iOS (Apple) mobile users will be provided to attendees approximately one week prior to the course. Within the app, attendees will have the opportunity to browse through slides, take notes, highlight and access additional course information including speaker bio information and local attractions.
