Healthcare delivery in the United States remains costly and fragmented, underscoring the need for strengthening the primary care medical home. Increasingly older and medically co-morbid patients strain capacity of primary and specialty care at a timw when payers are beginning to shift away from fee-for-service toward value-based repayment models. Integrated speciality care models in primary care practice may help acheive the aims of high value care delivery. This presentation will highlight the external and internal trends that have led to implementation of the Integrated Community Specialist (ICS) model in Employee and Community Health (ECH), as well as preliminary data on its impact to date.
