Target Audience

This course is designed for healthcare providers in family medicine.

Learning Objectives

Upon completion of this activity, participants should be able to:

  1. Identify how care team composition and continuity of care affects the efficient delivery of primary healthcare services and the quality of care patients receive.
  2. Explain how a case report can impact your awareness of unusual presentations, eliminate anchoring bias, and improve practice-based learning.
  3. Describe how the PDSA cycle and other QI techniques can be used to drive improvement in our delivery of preventative and chronic disease management services to our panel of patients.
