The Mayo Post-Acute Care Conference is intended for Mayo Midwest hospital care team members who participate in the Mayo Post-Acute Care program.  Leadership messages and patient stories will highlight successes of the program, and educational content for providers (MD, NP, PA, RN, PT, OT and Social Workers) working in transitional care units will be provided. Registration and attendance is by invitation ONLY.

Directions to Mayo Foundation House

  • From  2nd Street:  Turn south on 6th Avenue SW and drive two blocks to 4th Street SW.  Turn right (west) and drive one block.  Foundation House is the large stone house on the north side of the street, surrounded by a stone wall.
  • From Broadway:  Turn west on 4th Street SW and travel seven blocks.

Parking

  • There is no guest parking on the estate grounds except for handicapped approved vehicles.
  • Parking is available on most streets in the neighborhood surrounding the Foundation House.  Be sure to watch for signs that specify when parking is NOT allowed.
  • Employees with parking privileges (and tags) may use available parking stalls in Employee Parking Lot 17 (East of Foundation House) and Employee Parking Lot 16 (West of Damon House). Guest parking is allowed in these lots after 2:00 p.m.

Cancellation and Refund Policy:

Request for cancellations must be submitted in writing to cme@mayo.edu. When cancelling a registration for a course 14 days or more before the course start date, a full refund (minus a $75 administrative fee) will be issued in the same form of payment the registration was received. No refunds are granted less than 14 days before the course start date.
