DESCRIPTION
This activity occurs the fifth Thursday of the month, 7 - 8 a.m.
These case conferences inform and educate providers on how the organization responds to events. Each grand rounds may feature patient cases and/or multi-disciplinary approach to reduce harm and promote patients safety. Attendees are educated as to what actions were put into place to prevent the incidence from happening again. Commitment to Safety's Five Safe Behaviors are identified in each case as relevant.
TARGET AUDIENCE
This course is intended for all providers and allied health staff.

LEARNING OBJECTIVES
- Describe how a collaborative approach to patient care can help reduce patient harm
- Recognize the difference between a just culture environment and a non-punitive environment
- Describe how the root cause analyses process encourages transparency in reporting medical errors
- Cite cases where systems breakdowns contributed to patient harm
- Identify proactive strategic initiatives and their implementation
Each session may also provide it's own objectives.
