At the conclusion of this presentation, participants should be able to:
- Describe national aging demographics and their relationship to health care costs, regional variation, and new CMS reimbursement models.
- Define Time Driven Activity Based Costing and describe how that methodology can be used to demonstrate the value of care provided for specific populations in primary care.
- List at least three high value care models for high risk elders.
