Objectives:

  1. Define the principles of shared decision making (SDM)
  2. Identify a toll used to help assess stroke and bleed risks
  3. Utilize a SDM model to aid and augment education in the rehab setting
  4. Demonstrate familiarity with the PROMIS NIH outcome measure system
  5. Develop better understanding of patterns of patient referrals for MSK between Ortho, PMR and other non-surgical specialists
  6. Determine the utilization of this information to improve patient care and outcomes for PMR patients
