This Grand Rounds presentation will focus on Measurement Enhanced Care, an approach that can empower patients by making it possible for them to track their targeted symptoms, side effects, engagement in therapy and quality of life at visits and between visits. This will help patients and their families actively work with you to optimize their care. This information can be used to make decisions about what treatments to start, when to make modifications and once remission is reached how to avoid relapse.
