Mayo Clinic Practice Redesign Collaborative - COPD Hospital Readmission Reduction 2017

Starting in 2015, hospitals face penalties from the Centers for Medicare and Medicaid Services for excessive readmissions of patients with chronic obstructive Pulmonary Disease (COPD). It is estimated that as many as 79% of hospitals will face penalties. 21 percent of patients in the US with COPD are readmitted to the hospital. At Mayo Clinic, hospital readmittance is less than 10 percent. Mayo Clinic will share their best practices for reducing readmissions due to COPD. This will be diffused through a 1.5 day face-to-face, interactive conference in Rochester, Minnesota, April 11-12. After the face-to-face meeting, monthly coaching calls and webinars will be held to monitor implementation progress of the teams that attended the face-to-face meeting. Teams will be asked to gather metrics on their team's progress over the course of 6-9 months and report on their findings.
