Target Audience

This conference is designed for social workers, nurses, and dialysis care teams in kidney and pancreas transplant care.

Learning Objectives

Upon completion of this activity, participants should be able to: 

  • Describe the components of the transplant process from referral through to transplantation including the role of the care team and the patient journey.
  • Outline effective communication techniques for discussing transplantation with patients and families.
  • Identify barriers to timely transplant referral and strategies to overcome them. 
  • Integrate transplant education into routine dialysis care.
