This conference is designed for social workers, nurses, and dialysis care teams in kidney and pancreas transplant care.
Learning Objectives
Upon completion of this activity, participants should be able to:
- Describe the components of the transplant process from referral through to transplantation including the role of the care team and the patient journey.
- Outline effective communication techniques for discussing transplantation with patients and families.
- Identify barriers to timely transplant referral and strategies to overcome them.
- Integrate transplant education into routine dialysis care.
