The Mayo Post Acute Care Update is specifically designed for Mayo Midwest hospital care team members who actively participate in the Mayo Post Acute Care program. This publication features leadership messages and patient stories, showcasing the program’s achievements. Additionally, it provides educational content tailored for providers (including MDs, NPs, PAs, RNs, PTs, OTs, and Social Workers).
Target Audience
Physicians, Nurses, Nurseoractitioner, resident fellow, social worker, speech language pathologist, allied health professional.
Learning Objectives
Upon completion of this activity, participants should be able to:
- Apply a strategy to improve patient mobility and one method to assess cognition in post-acute care.
- Demonstrate de-escalation or safety technique for managing patients with behavioral challenges in low-resource settings.
- Explain a practical use of artificial intelligence in post-acute or critical access care settings.
- Identify actions that support personal or team well-being in the workplace.
- Describe key steps in emergency preparedness for critical access hospitals.
- Recognize one example of an innovative care model that can be applied in their setting.
