Family Medicine is evolving quickly to provide high quality care for a variety of patients throughout their lives. New team-based models of care are necessary to meet the quadruple aims of enhanced patient experience, improved population health, reduced costs, and improved work-life of clinicians. The Family Medicine Forum reviews the latest innovations surrounding chronic disease management, care team composition, continuity of care, and medical education. Original research, quality improvement projects, humanitarian community service, and medical education scholarship are highlighted via an interactive scientific poster session and several short oral presentations. The Family Medicine Forum is held in conjunction with Mayo Clinical Reviews, providing an evening CME opportunity and dinner reception for physicians, physician assistants, and nurse practitioners to network with colleagues.

Target Audience

This course is designed for healthcare providers in family medicine.

Learning Objectives

Upon completion of this activity, participants should be able to:

  • Identify how care team composition affects the quality-of-care patients receive.
  • Summarize how continuity of care affects the efficient delivery of primary healthcare services.
  • Explain how a case report can impact your awareness of unusual presentations.
  • Recall techniques used to eliminate anchoring bias.
  • Describe how the PDSA cycle and other QI techniques can be used to drive improvement in delivery of disease management services to our panel of patients.
