This course is designed for healthcare providers in family medicine.
Learning Objectives
Upon completion of this activity, participants should be able to:
- Describe how care team composition affects diabetic quality outcomes.
- Give examples of how pharmacist – physician collaboration can reduce total post-discharge cost of care for hospitalized patients.
- Demonstrate how greater organization of care with improved continuity of care can decrease hospital readmissions.
