The Mayo Post Acute Care Update is intended for Mayo Midwest hospital care team members who participate in the Mayo Post Acute Care program. Leadership messages and patient stories highlight successes of the program. Also, educational content for providers (MD, NP, PA, RN, PT, OT and social worker) working in transitional care units is provided.

Target Audience

This course is designed for allied health professionals, nurses, nurse practitioners, pharmacists, physicians, residents, fellows, and social workers.

Learning Objectives

Upon completion of this activity, participants should be able to:

  • Demonstrate current wound care techniques and apply them to patient care at the bedside.
  • Integrate palliative care techniques including care planning and symptoms management for patients needing palliative care at the MPAC sites.
  • Assess MPAC readmission rates, one year mortality rates, and readmission drivers and integrate this into transition planning at the MPAC sites.
  • Identify strategies to improve wellness and resiliency among MPAC staff.
  • Identify and describe the current and future roles of telemedicine in patient care at the MPAC sites.
