Advanced Practice Providers (APPs) play a central and expanding role in the longitudinal care of heart and lung transplant recipients, managing complex clinical decisions across inpatient and outpatient settings. Responsibilities include daily management, care coordination, protocol implementation, and early identification of clinical changes in a high-risk population. Despite advances in immunosuppression and transplant management, variability persists in diagnostic interpretation and application of evidence based practices. Contributing factors include evolving guidelines, emerging therapies, and variation in clinical experience, which can lead to inconsistent care, delayed recognition of complications, and suboptimal outcomes. Standardized, high-quality education tailored to APP practice is needed to reduce variability and strengthen clinical decision-making. Focused education in key domains—immunosuppression management, rejection surveillance, infectious disease, and long-term organ management—can improve consistency and patient care outcomes. APPs have distinct learning needs compared to physicians, with greater emphasis on workflow-driven, protocol-based, and operational aspects of care. They benefit most from case-based, practical education aligned with real-world clinical scenarios, including interdisciplinary communication and common pitfalls in post-transplant management. While designed for APPs, this program supports a multidisciplinary audience. Pharmacists, nurses, and allied health professionals are integral to transplant care, particularly in medication management, infection prevention, patient education, and care coordination. Addressing shared challenges—such as rejection identification and complex medication regimens—reinforces a coordinated, team-based approach to improving outcomes.
