The 31st Mendelson Advanced Facial Anatomy course focuses on a deeper understanding of the deep plane facelift anatomy. It will be an extended version of the renowned two-day MAFAC Course. This course is specifically oriented for surgeons who want to learn how to safely incorporate the latest advances in understanding facial anatomy into their aesthetic facial plastic surgery. The course is tightly structured as a guided, hands-on dissection program, having only two participants for each cadaver head, with a highly experienced dissection tutor for every four participants. The hands-on eyelid and brow rejuvenation component will focus on contemporary approaches to eyelid and brow rejuvenation including upper blepharoplasty, ptosis repair, lower blepharoplasty, and brow lifting.

Target Audience

This course is designed for surgeons in the field of plastic surgery.

Learning Objectives

Upon completion of this activity, participants should be able to:

  • Describe current concepts in SMAS anatomy, accessing the deep plane safely using the premasseter space and the prezygomatic space
  • Identify the zygomatic ligaments and zygomaticus major muscle
  • Describe extending the deep plane dissection safely into the neck; elevating the composite flap from its posterior attachments: the platysma-auricular ligament and the cervical retaining ligaments
  • Identify the orbital septum and tear trough ligament
  • Identify the levator aponeurosis
  • Integrate ptosis correction into upper blepharoplasty procedures
  • Integrate brow lift approaches

    • MARKINGS for safe and effective deep-plane surgery
    • INFILTRATION for hydro-dissection, anesthesia, and hemostasis
    • INCISIONS for inconspicuous scars
    • SUBCUTANEOUS UNDERMINING up to anterior border of parotid: “deep plane entry point”
    • Learning to identify the platysma-auricular fascia and the orbicularis oculi and platysma muscles

 

  1. DEEP PLANE DISSECTION (DR. FRESCHI & DR. MENDELSON)
    • Entering the deep plane safely using the premasseter & prezygomatic spaces
    • Identifying the zygomatic ligaments & nerve branches, and zygomaticus major (“red carpet”)
    • Connecting the two spaces safely to release the composite flap
    • Accessing the buccal fat pad

 

  1. DEEP PLANE LATERAL NECK (DR. FRESCHI & DR. MENDELSON)
    • Cervical retaining ligaments
    • Platysma myotomy vs. complete platysma transection
  2. SUTURE PLACEMENT (DR. FRESCHI & DR. MENDELSON)
    • Placement of key sutures to re-create the retaining ligaments.
    • Closing the SMAS & closing the skin without tension.
  3. DEEP PLANE TEMPORAL BROW LIFT (DR. FRESCHI & DR. MENDELSON)
    • Temporal dissection to efface facelift soft tissue redundancy, lateral hooding and tail of the brow.

    • Submental incision placement
    • Subcutaneous undermining with release of adhesions to the Mandibular Ligament
    • Undermining of platysma
    • Undermining of subplatysmal fat (“deep fat”) to reflect it inferiorly pedicled on strap muscles
    • Release of Peri-Hyoid Fascia & Identification of Digastric Tendon
    • Plication of Digastric Tendons to lift the Hyoid bone and sharpen the Cervicomental Angle
    • Tangential shaving of ADM and completing ADM plication – “Digastric Corset”
    • Incision of Capsule of Submandibular Glands (SMG)
    • Complete release of SMG within capsule
    • Reduction of superficial lobe of SMG – Identify & Ligate Central Perforating Artery!
    • Midline platysmaplasty and closing of the neck.
