The Mayo Post Acute Care Update is intended for Mayo Midwest hospital care team members who participate in the Mayo Post Acute Care program. Leadership messages and patient stories will highlight successes of the program, and educational content for providers (MD, NP, PA, RN, PT, OT, Pharmacist and Social Workers) working in transitional care units will be provided.

Target Audience

This course is designed for allied health professionals, nurses, nurse practitioners, pharmacists, physicians, residents, fellows, and social workers.

Learning Objectives

Upon completion of this activity, participants should be able to:

  • Recognize pressure ulcer staging and prevention.
  • Assess MPAC readmission rates, one year mortality rates, readmission drivers.
  • Identify strategies to improve wellness and resiliency among MPAC Staff.
  • Identify future roles of Telemedicine in patient care at the MPAC sites.
  • Demonstrate current wound care techniques.
  • Apply care techniques to patient care at the bedside
  • Integrate readmission transition planning at the MPAC Sites.
